Your First Visit

We ask that you arrive about 10 minutes early so that you will have enough time to fill out a New Patient form. For your convenience, you can also print and fill out our New Patient Form to bring to your appointment.

New Patient Form

Initial Examination

Your dentist will review your medical and dental history and complete a dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening, and any required X-rays.

Treatment Plan

Your dentist will examen your history and develop a customized treatment plan for your oral health needs. Your dentist will also be happy to answer any questions you have about your dental health, and suggest when you should have your next check-up based on your unique needs. 

Insurance & Financing

At Life Dental, our team will help you navigate our options for payment, insurance, and financing so you can focus on achieving your dental goals. If you have any questions about your insurance coverage or the methods of payment that we accept, please do not hesitate to ask one of our administrative staff members.

Insurance Claims

We can direct bill your insurance company and our administrative team is happy to assist you in understanding your insurance plan better. 

Payment Methods

The full treatment cost must be paid at the time of your appointment. We accept payment by cash, debit, or most major credit cards.
